New and returning members must complete
all forms in the registration packet. Please bring completed
forms to the first scheduled practice.
The Registration Packet includes:
- Membership Application
- Membership Fees
- Medical Information (Separate form required
for each participant; and signed by one parent)
- Waiver
of Liability (Separate form required for each participant;
and signed by one parent)
- Code of Conduct (Separate
form required for each participant; and signed by each
parent) all prices are updated
